Robotic Surgery

The development of oncology surgery has shifted toward minimally invasive techniques that prioritize reducing patient morbidity without sacrificing cancer-free outcomes.

Overview

The primary goal in modern Head and Neck oncology is "Organ Preservation"—curing the malignancy while safeguarding the patient’s ability to speak, swallow, and interact socially. Transoral Robotic Surgery (TORS) is at the forefront of this evolution, offering a sophisticated alternative to traditional open "jaw-splitting" surgeries.

The Technology: How TORS Works It Can Occur

The robotic system serves as a high-tech extension of the surgeon’s hands. It consists of three primary components:

• The Robotic Arms: Thin tubes equipped with miniaturized instruments that possess a greater range of motion and rotation than the human wrist, allowing them to navigate the tightest spaces of the mouth and throat.

• The 3D Endoscope: A high-definition, magnified camera that provides the surgeon with a depth-perceived, crystal-clear view of the tumour and surrounding healthy tissue.

• The Surgeon’s Console: A specialized workstation where the surgeon controls every movement of the instruments with extreme precision and filtered tremors.

Indications: Who is a Candidate?

TORS is highly effective for specific areas of the head and neck, typically paired with a neck dissection to manage lymph nodes. It is primarily recommended for early-stage cancers in the following regions:

• Oropharynx: Including the tonsils, base of the tongue, and soft palate.

• Hypopharynx & Supraglottic Larynx: The upper portions of the throat and voice box.

• Parapharyngeal Space: Tumours located in the deep tissues of the neck and throat.

Note

While TORS is a revolutionary tool for early-stage disease and organ preservation, it is generally not recommended for advanced-stage cancers where the tumour has extensively invaded deep structures, requiring more radical traditional approaches.
